Primary health care measurement framework and indicators: monitoring health systems through a primary health care lens

To continuously strengthen PHC, countries must be able to assess how decisions, actions and investments are addressing the broader determinants of health while improving service coverage, financial risk protection, and ultimately the health of individuals and populations. As countries strive to reorient their health systems around the principles of PHC, this document responds to Member States’ request in resolution WHA72.2 on primary health care for guidance to assess, track and monitor PHC performance to accelerate progress towards UHC and the health-related SDGs. The indicators and monitoring and evaluation conceptual framework presented in this document are based on and support the 14 levers of the Operational framework for primary health care. The indicators and framework have undergone technical review and multiple stages of consultation with countries, civil society, and leading PHC academics and experts.

Elements and Performance Indicators of Integrated Healthcare Programmes on Chronic Diseases in Six Countries in the Asia-Pacific Region: A Scoping Review

Globally, hospital-based healthcare models targeting acute care, are not effective in addressing chronic conditions. Integrated care programmes for chronic diseases have been widely developed and implemented in Europe and North America and to a much lesser extent in the Asia-Pacific region to meet such challenges.

This scoping review aims to examine the elements of programmes identified in the literature from select study countries in the Asia-Pacific, and discuss important facilitators and barriers for design and implementation.

Significant attention has been directed toward addressing health inequities at the population health and systems levels, yet little progress has been made in identifying approaches to reduce health inequities through clinical care, particularly in a primary health care context. Although the provision of equity?oriented health care (EOHC) is widely assumed to lead to improvements in patients’ health outcomes, little empirical evidence supports this claim. Defining indicators for assessing integrated care (2012-2014): Lessons learned on methodology and the evolution in assessment approach

Integrated care interventions are extremely complex as they tend to invilve multiple actors and different care levels. When evaluating such programmes indicators provide several benefits in comparison with other approaches. The Agència de Qualitat Avaluació Sanitàries de Catalunya, through a new collaborative approach, has been working on the development of indicators specially aimed at assesing integrated care. The aim of this study was to present the methodology developed and review the evolution of the prioritized indicators in three different projects aimed at assessing chronic integrated care initiatives.
