ENS4Care: Evidence Based Guidelines for Nurses and Social Care Workers for the deployment of eHealth services

The EU-funded Ens4Care project has developed five guidelines for European nurses and social workers on how to use eHealth for promoting a healthy lifestyle and prevention, clinical practice, skills development for advanced roles, integrated care and nurse ePrescribing.

The 2-year-project, that was launched in December 2013, funded by the EU and coordinated by the European Federation of Nurses Associations (EFN) together with 24 partners from all over Europe, including a mix of different professional associations in nursing and social care, nursing regulators and unions, informal carers, patients, researchers and research communities, civil society representatives and industry. They have collected a total of 122 existing good practices of using eHealth tools by nurses and social workers at both national and regional levels, with aim to inform not only the health and care sector, but also policy-makers.

Shifting acute care delivery from hospitals to homes in Ireland

Community-based acute care services delivered by mobile nursing teams were introduced by Caredoc, a private nonprofit organization contracted by the Irish Health Services Executive; first-hand insights of providers enabled identification of services delivery challenges and supported development of relevant solutions; supportive senior management generated momentum for change and helped secure necessary approval for activities from authorities; stakeholder engagement was described as time consuming, but crucial to success; electronic medical records and new technologies facilitated the creation of a simple, connected and user-friendly service; training for nursing staff was important for establishing the necessary clinical competencies to deliver acute care services in home settings.

Voices on person-centred care

What is person-centred care to a patient, a physician, a nurse, a hospital head of department, an industry representative and a researcher? How do these people, who all have experience of receiving or working with person-centred care, see person-centred care from their perspectives? Short film clips from the University of Gothenburg Centre for Person-centred Care - GPCC. GPCC is an in terdisciplinary research centre, established in January 2010, with the support of the Swedish government's strategic investment in health and care research. Uniquely GPCC has evaluated the effects of person-centred care in controlled studies. This has established that person-centred care is capable of enhancing the efficiency of the care process. Apart from considerable monetary savings resulting from amongst other things shorter hospital stays, significant positive effects from a patient perspective have been seen, as care is adapted to each individual person and his or her resources are made use of.

Partnership, Person-Centredness in Health Care

Person-centred care is an emerging concept in health care. Many people ask what is new about it, and as a matter of fact many health care professionals believe that they work in a person-centred way already. But shifting to a person-centred care practice often involves a profound change in culture and structure, and the key is a different approach in seeing the patient as a partner. The main thing is about being able to listen. Being able to listen to the patient and co-create a care plan; to create a partnership in health care.

The University of Gothenburg Centre for Person-centred Care - GPCC, performs research, education and innovation in person-centred care. Many of the centre’s projects are performed in partnership with Sahlgrenska University Hospital, one of the biggest in Europe, which is closely linked to the University via the The Sahlgrenska Academy.

Performance management in primary healthcare: Nurses’ experiences

The use of the performance management (PM) system is highly contested by public servants in South Africa, although its value as essential to the appraisal and management of staff is undeniable. Before considering the integration of a system or parts of a health system, a diagnosis must be made to assess how segmented it is. The vision of health professionals is essential to obtain a panoramic view of the situation.

The aim of study was to explore nurses’ perceptions and experiences of the PM system at primary healthcare (PHC) facilities in relation to the current health system’s reforms.

Associations between person-centred care and job strain, stress of conscience, and intent to leave among hospital personnel

Hospitals have experienced difficulty in retaining qualified healthcare personnel. Previous studies have shown that working in a person-centred environment could offset this challenge, but research is scarce. The organisational and environmental support subscale of perceived PCC showed significant correlations with all explanatory variables, while the extent of personalising care subscale only correlated with job strain and intent to leave. Higher perceived PCC is associated with work-related factors in hospital departments.

How do three components of professional commitment influence nurse-reported patient-centred care and care quality?

Patient-centred care and care quality are the two critical care outcomes. However, no study has yet examined how the three components of professional commitment are related to nurse-reported patient-centred care and care quality, showing a research gap. This study shows that affective professional commitment is positively associated with nurse-reported patient-centred care, which is positively associated with nurse-reported care quality.

Nurses' experiences of person-centred care planning using video-conferencing

The aim was to illuminate how nurses experience person-centred care planning using video conferencing upon hospital discharge of frail older persons. This study provides valuable insights into challenges associated with care planning via video conferencing. The meeting format, that is video conferencing, is perceived as a barrier that makes the interaction challenging. Shortcomings in video technology make a person-centred approach difficult. The person-centred approach is also difficult for nurses to maintain when the older person or relatives are not involved in the planning.

Nursing Care Coordination in Primary Healthcare for Patients with Complex Needs: A Comparative Case Study

Despite nurses’ substantial role in care coordination, few education programs exist to better support them in this role. Identification of a set of core care coordination activities across heterogeneous care coordination programs would facilitate the development of a standard of practice. We sought to examine care coordination activities across two care coordination programs in Family Medicine Groups in Quebec, and their relationship to the program design.

The organisation of primary health care service delivery for non-communicable diseases in Nigeria: A case-study analysis

As chronic diseases, non-communicable diseases (NCDs) require sustained person-centred and community-based care. Given its direct link to communities and households, Primary Health Care (PHC) is well positioned to achieve such care. In Nigeria, the national government has prioritized PHC system strengthening as a means of achieving national NCD targets. However, strengthening PHC systems for NCDs require re-organization of PHC service delivery, based on contextual understanding of existing facilitators and barriers to PHC service delivery for NCDs.

Patient-centered care and geriatric knowledge translation among healthcare providers in Vietnam: translation and validation of the patient-centered care measure

People are living longer, and the majority of aging people reside in low- and middle-income countries (LMICs). However, inappropriate healthcare contributes to health disparities between populations of aging people and leads to care dependency and social isolation. Tools to assess and evaluate the effectiveness of quality improvement interventions for geriatric care in LMICs are limited. The aim of this study was to provide a validated and culturally relevant instrument to assess patient-centered care in Vietnam, where the population of aging people is growing rapidly.
