The Urgent Care: Improving Patient Flow conference agenda has been developed to explore the current patient flow trends within the hospital urgent care system. Delegates will gain a greater understanding of how the problems occur from the start of the process, with the patient having attended the A & E department, through to the discharge process, returning back into the community and home. Drawing on the findings from recent reports by The King’s Fund, Healthwatch England, the National Audit Office, the Care Quality Commission and the Parliamentary and Health Services Ombudsman (PHSO), this event will explore new ways of ensuring older people experience a safe, appropriate and timely discharge that is right for them and their health needs. In this event you will discover good practice examples in health, local government, social care, community services and housing and learn how services can plan ahead across their whole populations to proactively manage those most at risk.

The current focus on new care models, and place-based systems of care, requires organisations to work together to deliver services. How well these new systems are led, directed and held to account will be crucial to their success. This one-day conference will help those who are considering how governance and accountability will work in these new, place-based systems.

This conference will explore the ways NHS organisations and their partners can collaborate in order to meet the needs of the population they serve. Showcasing learning from national and international accountable care systems, as well as other emerging new care models, conference sessions will provide a key opportunity to hear and learn from local areas on the journey towards more systemic and integrated approaches to improving population health.

The primary and acute care systems (PACS) model and multi-specialty community provider (MCP) model are particularly focussed on building partnerships between GP, hospital, community and mental health care organisations to build a health care system to meet their local population’s health needs.

This Conference will take a detailed look at each of these models of care. The ICARE4EU project, financially supported by the Health Programme 2008-2013 of the European Commission, wants to improve the care for people suffering from multiple chronic conditions in Europe: Sharing knowledge and experiences to develop new policies and practices.

The conference was focused on the ways in which integrated care can be delivered for older people with frailty, looking at interactions between acute, primary, community and social care services, older people living with complex co-morbidities and ways in which older people with frailty can be supported to maintain their health and wellbeing.
