Evaluating a Community-Placed and Clinically Integrated Community Health Worker Program

Community health worker (CHW) programs can act as bridges between patients and health care teams, but the complexity and variability of program components and outcomes make them difficult to evaluate. This evaluation used a realist approach to identify underlying mechanisms and contextual factors associated with successful implementation of a community-placed CHW program affiliated with a primary care practice in the Midwest United States. The analysis identified mechanisms by which stakeholders built trust, self-efficacy, and empowerment to improve patient-centered outcomes and experiences.

A compendium of tools and resources for improving the quality of health services

In an attempt to consolidate the work of the WHO Department of Service Delivery and Safety (SDS) on quality improvement, a SDS cross-cut team has produced a compendium of tools and resources on quality improvement, developed within the SDS, that are applicable for country support. It includes resources on IPCHS and community engagement among others. Community-Based Social Innovations

This research defined CBSIs as community initiatives that seek to: a) empower older people to improve their self-efficacy in caring for themselves and their peers; b) maintain their well-being and; c) promote social cohesion and inclusiveness. Health systems across both developed and developing regions struggle to meet the diverse and complex needs of increasing number of older persons. This study hypothesized that community based social innovations (CBSIs) can improve the health and well-being of older people through community-based care.

Project INTEGRATE: Developing a Framework to Guide Design, Implementation and Evaluation of People-centred Integrated Care Processes

People-centred integrated care is an acknowledged approach to improve the quality and effectiveness of health systems in delivering care around people’s needs and preferences. Nevertheless, more guidance on how to effectively design, implement and evaluate the care process of people-centred integrated care services is needed. Under Project INTEGRATE, a framework was developed to guide managers in the assessment, transformation and delivery of these health service innovations.

Why integrated care already owns the future

Integrated care has certainly arrived in the primary care clinic. Evidence and momentum began building 20 years ago with the demonstrated effectiveness of collaborative care programs for depression and anxiety disorders in primary care. Since then, various forms of integrated care have been repeatedly proven effective across a range of care settings, patient populations, languages and cultures.

Integrated care models are well adapted to thrive during the Fourth Industrial Revolution in health care. Because integrated care has embraced flexibility and disruption, integrated care models will succeed—or even dominate—in a future that will demand flexibility.

The multispecialty community provider (MCP) emerging care model and contract framework

Across the country, NHS leaders have been developing sustainability and transformation plans (STPs) to implement the NHS Five Year Forward View. Nearly all of the STPs involve creating new models of accountable care provision. Integrated primary and acute care systems (PACS) - Describing the care model and the business model

A new framework to help local areas establish a single primary and acute care system (PACS) has been published by NHS England. Under a PACS, hospital, primary, community, mental health and social care services align their goals and incentives to improve the health and wellbeing of the population.

The announcement comes following the publication in July of a framework for the multispecialty community provider (MCP) model.

A guide to enabling the spread of person- and community-centred approaches for health and wellbeing

This guide is addressed to people who champion person- and community-centred approaches in health and social care. It is part of the NHS England-funded Realising the Value programme led by Nesta and the Health Foundation, which seeks to develop person- and community-centred approaches for health and wellbeing. Two main contents are included in the guide: a framework for understanding and changing behaviour, and real-world examples of how these changes happen in practice.
