The COVID-19 pandemic puts health and care systems under pressure globally. This current paper highlights challenges arising in the care for older and vulnerable populations in this context and reflects upon possible perspectives for different systems making use of nested integrated care approaches adapted during the work of the EU-funded project VIGOUR (“Evidence based Guidance to Scale-up Integrated Care in Europe”).

Clinicians’ experience of providing care: a rapid review

Health care services internationally are refocussing care delivery towards patient centred, integrated care that utilises effective, efficient and innovative models of care to optimise patient outcomes and system sustainability. Whilst significant efforts have been made to examine and enhance patient experience, to date little has progressed in relation to provider experience. This review aims to explore this knowledge gap by capturing evidence of clinician experience, and how this experience is defined and measured in the context of health system change and innovation.

Supporting Older People to Live Safely at Home – Findings from Thirteen Case Studies on Integrated Care Across Europe

While many different factors can undermine older people’s ability to live safely at home, safety as an explicit aspect of integrated care for older people living at home is an underexplored topic in research. Integrated care services across Europe address older people’s safety in many ways. Further integration of health and social care solutions is necessary to enhance older people’s perceptions of safety.

Patient autonomy in the consultation: How signalling structure can facilitate patient-centred care

This paper explores how patient autonomy may be supported or undermined in doctor-patient interactions through sharing information about what is to happen in the consultation. Doctors use a variety of verbal signalling behaviours with multiple functions. As well as sharing information, these behaviours regulate patient agency in the consultation.

Patient feedback to improve quality of patient-centred care in public hospitals: a systematic review of the evidence

Public health services have been moving towards putting patients at the centre of their care. This paper aimed to review systematically the published literature relating to interventions informed by patient feedback for improvement to quality of care in hospital settings. Multi-component interventions which targeted both individual and organisational levels were more effective than single interventions. Care is truly patient-centred when it is guided by the perspective of the one that matters - the patient.

Quantitative evaluation of an outreach case management model of care for urban Aboriginal and Torres Strait Islander adults living with complex chronic disease: a longitudinal study

Chronic diseases are the leading contributor to the excess morbidity and mortality burden experienced by Aboriginal and Torres Strait Islander (hereafter, respectfully, Indigenous) people, compared to their non-Indigenous counterparts. The Home-based Outreach case Management of chronic disease Exploratory (HOME) Study provided person-centred, multidisciplinary care for Indigenous people with chronic disease. This model of care, aligned to Indigenous peoples’ conceptions of health and wellbeing, was integrated within an urban Indigenous primary health care service.

The clinical effectiveness of the COPDnet integrated care model

COPD is a disease with a high prevalence in the world. Despite the great personal and family involvement, it has been seen that integrated care between hospitals and primary care translates into better clinical results for patients. Integrated care models have the potential to improve outcomes for patients with COPD. The research team design the COPDnet integrated care model and implemented it in two hospitals and affiliated primary care regions in the Netherlands. The COPDnet model consists of a comprehensive diagnostic trajectory ran in secondary care followed by a non-pharmacological intervention program of both monodisciplinary and multidisciplinary components.

Effective implementation approaches for healthy ageing interventions for older people: A rapid review

Given the advanced demographic transition in the world, seeking healthy aging is an imperative task. In this review, strategies that favor a better quality of life for the elderly are shown, including the interaction of different levels of care (intra and extra system). All of this results in better global health indicators for the elderly population, which in turn translates into less need for care by the system.

Lessons on the COVID-19 pandemic, for and by primary care professionals worldwide

During the early stage of the pandemic, primary care continued as the first point of contact to the health system but was poorly informed by policy makers on how to fulfil its role and ill equipped to provide care while protecting staff and patients against further spread of the infection. The pandemic presents important lessons to strengthen health systems through better connection between public health, primary care, and secondary care to cope better with future waves of this and other pandemics.

Approaches to integrating palliative care into African health systems: a qualitative systematic review

Africa is characterized by a high burden of disease and health system deficits, with an overwhelming and increasing demand for palliative care (PC). Yet only one African country is currently considered to have advanced integration of palliative care into medical services and generalized PC is said to be available in only a handful of others. The integration of PC into all levels of a health system has been called for to increase access to PC and to strengthen health systems.
