This report, published by the Royal Pharmaceutical Society in Scotland, argues that pharmacist led care of people with long term conditions could deliver better results and be more cost effective. It focuses on the role of the pharmacist as part of a multidisciplinary approach to tackling the challenges facing the NHS in providing the highest quality care and support for people with long term conditions.

The NHS have worked with The King´s Fund to translate their themes into outcomes for patients and set out the most important priorities for action to achieve these outcomes. Under each of their five themes:

Many child deaths in developing countries are preventable: Children die from treatable conditions, such as pneumonia, diarrhea, and malaria, because families in rural, hard-to-reach, or conflict-ridden areas can’t access or afford the treatments. Community health care services provide vital care out of hospital for millions of people. From children’s services to care for older people and end-of-life support, the community sector plays a key part in meeting the challenges facing our health and care system.
