The challenges posed by CO VID-19 present an opportunity to rethink fragmented health and depende ncy care systems so that they are integrated\, driven by individuals a nd communities\, and resilient to future systemic crises.
\nThe speed and scale of response required by the COVID-19 pandemic shows ho w fragmentation in the health and dependency care systems significantl y impairs their effective response capacity. The provision of health s ervices and long-term care is generally compartmentalized. Inpatient a nd outpatient care is often separated from primary\, rehabilitative\, and home care that are provided after patients leave the hospital. The re is also no integration with the long-term care provided to people w ith functional dependency.
\nDuring this webinar\, IFIC&rsquo\;s very own Dr. Edelweiss Aldasoro will present an overview of integrate d care and the recent IFIC report &ldquo\;Realising the true value of Integrated Care\: Beyond COVID-19&ldquo\;. \; After this presentat ion\, practice-based examples that allow for better care linking\, coo rdination and integration will be examined. First\, Fatima Palmeira Bo mbarda\, Manager of Healthcare Networks in the Health in Action Progra m of the Sã\;o Paulo State Health Secretariat\, and Anne Hendry\ , Senior Associate and Director of the IFIC center in Scotland\, will present the experience of integrated care in the development of a Regi onal Medical Care Network in four municipalities (Ubatuba\, Caraguatat uba\, Sã\;o Sebastiã\;o and Ilhabela) in the North Littora l region of Brazil. Finally\, Galileo Pé\;rez Herná\;ndez\ , member of the IFIC Board\, will analyze the implications for current and future integrated care projects in Latin American countries using scientific implementation lenses and discuss their relationship with the concept of resilience of health systems.
